Legal or public name of your clinic, practice, or hospital.

Step 4 – Credentials & Verification

If Doctor / Physician

Enter your professional license number. DGP / SEP
Specialty credential (if applicable) DGP / SEP / CONACEM

If Clinic / Hospital

Enter your COFEPRIS registration or license number.

Step 5 – Banking Information (for Wire Transfers)

Legal entity name on the account.
Name of the financial institution.
Enter your 18-digit CLABE (for Mexico) or account number.
Required for international transfers.
Provide complete transfer instructions (Wise Business preferred).
